It is not necessary that an Individual needs to submit this Pledge Form to an Eye Bank, duly signed by him, essentially before his death. It is possible that even if such Form is not submitted to Eye Bank, still close relatives of any deceased person can take a decision to Donate Eyes of a deceased, fill this form (preferably two immediate relatiaves with two Witnesses) attach Death Certificate to the Form and an Eye Donation can be done, which is a Noble cause.
